Lean Six Sigma Process Improvement Templates

Practical tools to help you improve processes, solve problems, and achieve measurable results.

At Florida Lean, LLC, we believe Lean Six Sigma is best learned by applying it to real-world problems, not simply completing a course or passing an exam.

We've made these DMAIC templates available to help you organize your process improvement projects, document your findings, and work through each phase of the Lean Six Sigma methodology.

These are not the only tools or templates available in Lean Six Sigma.

Every process improvement project is different, and the tools you select should depend on the problem you're trying to solve. The resources below provide a practical starting point to help you get moving.

Define Phase- Identify the Problem

What problem are we trying to solve?

The Define phase establishes the foundation of your process improvement project. Before collecting data or proposing solutions, you need to clearly understand the problem, why it matters, who is affected, and what success should look like.

During this phase, you'll establish the project's scope, identify stakeholders, define the problem and goal statements, and develop a high-level understanding of the process.

The templates below will help you organize your project, establish clear objectives, and ensure everyone understands what you're trying to accomplish.

Measure Phase- Understand the Current Process

How is the process performing today?

The Measure phase moves your project from assumptions to facts. This is where you examine how the process actually operates, collect relevant data, identify waste, and establish a baseline of current performance.

Understanding where you are today is essential before determining what needs to change.

The templates below will help you document the current process, collect meaningful data, identify inefficiencies, and establish measurable performance baselines.

Analyze Phase– Identify the Root Causes

Why is the problem happening?

The Analyze phase focuses on understanding why the process isn't performing as expected. Rather than immediately implementing solutions, you'll examine the data, investigate potential causes, and determine which factors are actually contributing to the problem.

An important part of this phase is distinguishing between symptoms and root causes. Identifying a potential cause isn't enough; whenever possible, it should be supported by evidence.

The templates below will help you investigate potential root causes, organize your findings, and confirm the factors contributing to poor process performance.

Improve Phase– Develop and Implement Solutions

What changes will improve the process?

The Improve phase is where your findings turn into action. Using the root causes identified during the Analyze phase, you'll develop, evaluate, test, and implement solutions designed to improve process performance.

The goal isn't simply to change how work gets done. It's to make improvements that directly address verified root causes and produce measurable results.

The templates below will help you evaluate potential improvements, document proposed changes, plan implementation, and measure the impact of your solutions.

Control Phase– Sustain the Improvements

How do we make sure the improvements last?

The Control phase focuses on maintaining the gains achieved during your improvement project. Even a successful solution can lose its effectiveness if the organization doesn't establish a method for monitoring performance and maintaining the improved process.

During this phase, you'll establish process controls, assign responsibilities, document updated procedures, and determine how performance will be monitored over time.

The templates below will help you establish accountability, monitor process performance, and reduce the risk of returning to old habits.

A Starting Point, Not the Entire Toolbox

Lean Six Sigma offers numerous tools and techniques, and not every project requires the same ones.

The templates provided here are intended to help you get started with the DMAIC methodology. They are not an exhaustive list, nor should every template be considered mandatory for every project.

Remember: The goal of Lean Six Sigma isn't to complete templates. It's to improve processes.

Choose the tools that help you understand the problem, make informed decisions, verify improvements, and sustain the results.

At Florida Lean, LLC, we emphasize practical, project-based Lean Six Sigma training because knowing how to fill out a template is very different from knowing how to apply it to an actual workplace problem.

Learn the methodology. Apply the tools. Improve a real process.
